AGEWELL SERVICES MEDICAL TRANSPORTATION
The AgeWell Services Senior Transportation Program provides door-to-door, non-emergency medical transportation for low income Muskegon County seniors. The service operates Monday-Friday from 8:30am – 5:00pm. For more information, call (231) 726-7090. Eligibility Requirements: You must be 60 years or older, you must be at 150% of the poverty level or less. Service Area: We provide medical transportation in the Muskegon County in Michigan.

Network West Michigan is a Center for Independent Living, which is a unique organization with the goal of helping individuals achieve and maintain independence in their communities. We believe that people with disabilities themselves can make the best decisions for their own care and lives. We work with persons with disabilities to help them live as independently as possible and to increase accessibility in our communities. At minimum, 51% of our staff and board of directors are people with disabilities. This model empowers our ability to connect with those we serve and provide consumer & community driven services. One primary focus of our Center is to change the system; not the person with a disability. With nearly 20% of the population, or 1 in 5 people, experiencing a disability, we strive to ensure our communities promote diversity and inclusion. With this aim we strive to ensure that public spaces and opportunities are just as accessible to people with disabilities as everyone else and provide support and consultation services for community businesses and service providers. We are the only private non-profit agency that assists people with any disability of any age and just celebrated 20 years of serving our five county service area of Muskegon, Oceana, Newaygo, Mason and Lake and provide services to approximately 3,000 individuals per year. Sample of services offered: • Work to increase job placement for veterans with disabilities • Promote economic self-sufficiency for all persons with disabilities • Offer independent living skills training • Advocate for policy change on a local & state level • Provide services to youth in schools for post-secondary education/ job development opportunities • Provide ADA (Americans with Disability Act) consultation services to businesses and service providers • Increase the education and opportunities for individuals to return to work who are receiving SSI/ SSDI • Transition eligible individuals out of Nursing Facilities to community based settings • Provide transportation programs to people with disabilities living in rural areas
